With Microsoft Forms, you can create survey forms and share them easily with your friends, users or colleagues.
If you work as a team, you need to collaborate with your employees and share your resources with them in a timely manner. For example, if you create a survey to evaluate respondents’ responses, you must collect data and create a form. This data can then be processed and used for analysis and evaluation purposes. Microsoft Forms offers all these features with your Microsoft Office 365 subscription.

You’re going to have to start with a good plan: What are your needs? What types of data should be recorded? How will you present the decisions? These are questions that you must answer in advance. In our example, we create a simple form that recipients can use to report volunteer work. You will answer the following questions:
Last Name: Text
First name: Text
Volunteer Date: Date
Hours: text message
Remember that Microsoft Forms is a simple and limited feature, so don’t plan anything too complex.
1. The first step is to log into your Microsoft Office 365 account.
2. click on the application form.
3. click on the New Form or New Questionnaire button to start. I will create a new form to obtain feedback on clients’ experiences when they visit one of our facilities.
4. Enter a title name for your form (up to 90 characters).
5. Enter a subtitle (optional and up to 1000 characters).
6. Click on the Insert Image button to add an image to your form (optional).
Changing form settings
To access the Settings screen, click the Ellipses button (…) and select Settings from the list of displayed options.
Now select or delete the default settings on the Form Settings page.
As with the above setting, you can change or delete the default settings on the Quiz page.
If necessary, you can also customize the default message that recipients see when they try to send their response after disabling this setting.
The quality of a form for a quiz or survey depends on the information it can provide. Microsoft forms provide a summary of how your audience responded to the questionnaire or survey.
